You must complete all Occupational Health information honestly and accurately in order to comply with your employer’s Occupational Health requirements:

 

 

 

PLEASE READ THIS INFORMATION CAREFULLY

As a registered healthcare professional, you will understand the importance of ensuring that your health, and the health of others, is protected.   You will need to complete the following four requirements:

  1. Completed Occupational Health Questionnaire
  2. Secure evidence of relevant vaccinations and updated vaccination certificates (if applicable)
  3. TB Clearance from IOM (International Office of Migration) for visa application

 

1. OH Questionnaire –

The OH questionnaire will be sent to you either

  1. Emailed to you by our ASC Processing Specialist or
  2. Via a link sent directly from your employer to your personal email. You must complete the Trust’s OH Questionnaire honestly and accurately. 

    NHS Trust that sends OH link instead:
    - Barts Health NHS Trust
    - Isle of Wight

    - New Castle Upon Tyne Hospital
    - Nuffield Health

    - Royal Surrey NHS Foundation Trust - The Trust have requested for all candidates to enter onto the OH link - sent by their OH provider - Job Title as ‘Overseas Staff Nurse’ and not ‘Staff Nurse’.
    - The Children's Trust

2. Vaccination Summary Form – 

You must complete this form with your full vaccination history and submit to your ASC Processing Specialist with evidence, e.g. vaccination certificates. Certification from a doctor on a prescription pad is also accepted.

 The vaccinations you will need to evidence are:

  • MMR 1 & 2

ASC will advise if there are any further vaccination needed based on the result of your medicals. A certificate must be provided if in case an additional vaccination is required.

3. Medical Summary Form –  Download and to be submitted to our accredited diagnostic clinic/centers listed below

 

 

You also need to undergo the following tests: 

Chest X-ray 

IMPORTANT :If you have previous history of TB or aware of any scarring, there is no need to be apprehensive about it, we highly encourage and require for you to bring and present old TB documentation to IOM during your visit. Disclosing these past records will potentially help your screening physician in giving you proper recommendation.

TB Clearance at IOM (International Office of Migration) in Manila

  • A TB clearance certificate from IOM is a mandatory requirement for your UK visa application.
  • Your IOM appointment booking will be planned and managed by ASC. Please DO NOT book your flight to Manila until you receive your confirmation email from IOM or your ASC Processing Specialist
  • Please take your original passport (valid more than 6 months) and 2 valid IDs to your IOM appointment.  Chest X-ray results are released within the day. You may be required to complete additional tests, e.g. sputum specimens for culture and sensitivity (at no additional cost).  Sputum test results may take 8 to 9 weeks.
  • Please submit your IOM certificate, to your ASC Processing Specialist. 

If chest x-ray is marked “Abnormal”, ensure you obtain the CD of your chest x-ray before leaving the IOM clinic. You must declare any TB history and take with you any documentation you may have relating to previous infection or treatment.

It is advisable to obtain IOM TB clearance as soon as possible after receipt of your job offer for the following reasons: 

  • Anyone diagnosed with active TB will not be given a clearance certificate to proceed with visa application to the UK.
  • Anyone requiring sputum testing and/or treatment, may be able to complete this without affecting your planned deployment timeline.
  • Please note that IOM TB clearance certificates are usually only valid for 3-6 months. You will need an in-date/valid clearance certificate from IOM at the time of your visa application.
  • Early detection of infection and/or the need for sputum test will then allow time for possible treatment to be undertaken during the early part of your application process. This will in turn avoid any unnecessary delays to your deployment at the appropriate time.
  • This way, if you need additional testing or treatment, you may be able to complete this before your estimated deployment date.
